Contract [PLEASE READ}
This is a contract between MESAW, LLC (d/b/a Club Barks) (hereinafter referred to as "Club Barks") and pet Owner whose name appears below (hereinafter called “Owner”).
Owner understands that participation in daycare, boarding, training and grooming at Club Barks is not without some risk. Despite all of the dogs appearing healthy and being handled with the greatest amount of care and foresight, dogs are not always predictable and the unexpected may occur. Owner herby waives and releases Club Barks, its employees, the Owners and agents from any and all claims, damages, liabilities, suits or losses of any nature that arises or occur with the Owner or his dog or on the grounds or the surrounding area of Club Barks or resulting from participation in Club Barks, including specifically, but not limited to, any injury or damage resulting from the action of any dog(s). Owner further agrees to pay any medical veterinary expense incurred or as a result of the injury occurring on the property or being caused by any confrontation among dogs while in the care of Club Barks. Owner gives Club Barks permission to seek veterinary care at the veterinarian of their choice as they deem necessary for Owner’s dog(s) at Owner’s expense.
By signing this, I also understand the risk that my dog may develop a cough, runny nose, sneezing, canine influenza, parasites or stomach virus amongst other ailments that may stem from playing in a cageless environment and/or exposure to the other animals at Club Barks. Owner herby waives and releases Club Barks, its employees, Owners and agents from any and all claims, damages, liability, suits or losses of any nature that arise or occur to the above-mentioned or any other ailments.
Terms:
Payment is required when service is rendered. A 50% deposit may be requested to hold overnight boarding reservation. If you must cancel for any reason, we must receive notice (48) hours prior to the day of the reservation in order for your deposit to be refunded. No-shows and cancellations after this time are NON-REFUNDABLE. We accept cash, checks, Visa, MasterCard and American Express.
Agreement:
I agree to make prompt and complete payment upon discharge of the animal. I understand that if I neglect to pick up the above animal within (6) six days of notification to the above address, that said animal would be considered abandoned, and that doing so does not relieve me from financial obligation.
I further understand that in the case of non-payment, I will be subject to all billing finance and/or collection charges associated to my account.

By signing this registration form, you indicate that you are the owner, or the agent of the owner, of the aforementioned pet, and are authorized to release the pet to the care of Club Barks.
You also indicate that you understand the policies, procedures, and schedule of fees as stated on Club Barks website, and affirmatively, voluntarily and expressly consent to the release and other terms set forth in this contract.
Club Barks reserves the right to add to, update, and change these policies, procedures, and schedule of fees from time to time without notice. Continued use of our services after any such changes shall constitute your consent to such changes.
